Thank you for choosing Vendetti Wellness Group!  We recognize you have many choices and we look forward to working with you! 

 

Prior to your initial appointment you will receive a welcome email with information about Vendetti Wellness Group, your insurance eligibility if applicable and intake paperwork that you can download, print, review and sign before our first appointment. If you do not have a chance to obtain the intake paperwork prior to your first appointment we will use some of the time during this appointment so you can fill it out and to go over it.

Please bring with you the following:

• Intake paperwork printed and signed, intake forms can be found on our Intake Forms page

• Your primary insurance card

 

LOCATIONS

We now have two convenient locations in order to accommodate our clients! 

• Hayden Rowe Business Center at 34 Hayden Rowe Street - Suite 194, Hopkinton, MA 01748

• COMING APRIL 2018 Highland March Business Center at 20 Cabot Boulevard - Suite 300, Mansfield, MA 02048

PARKING

Both of our locations have plenty of parking located in the rear of the building(s).

 

MAIN ENTRANCE

Hayden Rowe Business Center Location:

The main entrance is located in the rear of the building.  Upon entering, please follow the staircase down.  You will most likely be greeted by the receptionist.  Please have a seat in the waiting room and you will be greeted for your appointment.  There is an additional entrance located on the right side of the building if ramp access is needed.  *Please let us know if you need access to the ramp entrance.

Highland March Business Center Location:  COMING APRIL 2018

The main entrance is located in the front of the building where you see the number 20 on the building.  Go up the stairs and upon entering the building, you shall see a set of elevators to your left.  Go up one level, to the 3rd floor.  Upon exiting the elevator, you shall see Suite 300 on your right.  Enter the suite and you will most likely be greeted by the receptionist.  Please have a seat in the waiting room to the left of the reception area and you will be greeted for your appointment.  There is an additional entrance located on the right side of the building if ramp access is needed.  *Please let us know if you need access to the ramp entrance.

 

INITIAL APPOINTMENT

Your initial appointment will be 60 minutes. We will explore your concerns, gather some background information and see how our services may help you achieve your goals.
